Pay: (IB68 - IB72) $38.14 - $40.52 per hour / (YP2 - YP9) $42.61 - $54.41 per hour
Location: Grampians Health Horsham (20 bed sub-acute facility)
About the Role
Grampians Health Horsham is a 20 bed sub-acute facility that provides admitted rehabilitation, geriatric evaluation and management (GEM), transition and palliative care services within a multidisciplinary team and patient focused environment.
Work Schedule
7‑day, 24‑hour rotating roster including day, afternoon, and night shifts. Permanent full‑time, part‑time and casual positions available.
Key Responsibilities
* Deliver individualised nursing care as part of a multi‑disciplinary team
* Liaise and effectively communicate with relevant medical, nursing and allied health team members
* Promote evidence‑based practice that has a problem‑solving approach
* Encourage patients, carers and families to participate in all stages of care
* When appropriate, supervise and direct clinical practice of junior Registered Nurses, Enrolled Nurses, and undergraduate nursing students and provide feedback
Skills & Experience
* Registered Nurse with current AHPRA registration
* Excellent communication and problem‑solving skills
* Knowledge of contemporary nursing theory and practice in relation to the care of sub‑acute patients/clients
* Thorough understanding of quality systems and health services accreditation

Additional Information
All appointments are subject to a satisfactory police records check, immunisation clearance and Working with Children check and/or NDIS Worker Screening Check.
All employees must be willing and able to wear Personal Protective Equipment (PPE) when required, which at a minimum is a surgical mask. Additional PPE requirements are role‑dependent.
