**What YOU do matters**
- Benetas has an exciting opportunity for an individual whom is highly organised, has proficient communication skills and who will support clients in navigating their Home Care Package. As a
- **Client Advisor (Case Manager)** you will be based out of our
- ** Bendigo Office** reporting to the Operations Team Leader. Your focus will be to develop a positive client experience in collaboration with the Rural North West Community Health and Care Team.
**Your Key responsibilities will involve;**
- Assist clients in understanding the requirements of their package and the choices available to them.
- Coordinate all services and supports in line with client requirements and needs.
- Undertake client intake and assessments, including care planning, monitoring and regular reviews.
- Provide a strength based and person centred approach to ensure client's independence is maintained.
- Guarantee all services delivered are consistent with the funding, guidelines and contracted requirements.
- Comprehensive documentation; case notes, care plans, correspondence and referrals
- Forward thinking and planning to maintain and prioritise adequate allocation of resources.
- Implement best practice quality models of assessment, care plan development, implementation and review.
- Actively promote and refer clients to Benetas services
**To thrive in this role you will have**:
- Excellent customer service and interpersonal skills, with the ability to demonstrate empathy and understanding to older clients and their families
- A positive approach to working within a team and autonomously
- Demonstrable experience in case management within aged care, home care, or allied health services (desirable)
- Tertiary qualifications in a health or social services-related discipline or related experience (desirable)
