About the Role
Concord Repatriation General Hospital has a well-earned reputation for excellence in healthcare for over 70 years. Building on its proud heritage of caring for the Veteran community, the hospital now services the local communities of Concord, Strathfield, Burwood and beyond.
The SLHD Community Transition Aged Care Program serves to transition aged care clients safely from hospital back into the community. The TACP Support Worker, in conjunction with the TACP team, will liaise with clients and team members to ensure Transition care is meeting the needs of the client.

Ideal Candidate
* Demonstrated experience monitoring clients and linking in with appropriate services
* Well-developed written, verbal and interpersonal communication skills
* Demonstrated computer skills with capacity to learn and use electronic reporting systems
* Current NSW Drivers Licence (P1 and P2 acceptable).
