Role Overview
The Practice Nurse Team Leader will ensure high quality, safe and efficient nursing services within the Marion Medical Centre.
Key Responsibilities
* Direct patient care activities:
o Conduct direct patient care in the treatment room following company Standard Operating Procedures (SOPs) and or Standing Orders.
o Administer vaccinations, medications, ECG, spirometry and other diagnostic tests, wound care, bandaging, plastering, sutures removal, triage, and emergency management.
o Perform pre‐employment medical screening and workcover injury management and assist with HCP‐led procedures.
* Chronic disease management:
o Assist the GP with health assessments, GP management plans, team care arrangements, returns and initiate other chronic disease management tasks (eg. domiciliary medication management reviews) per Medicare requirements.
o Provide a framework and support system to coordinate care for patients with chronic conditions.
o Ensure collaboration with clinical and non‐clinical support teams for a holistic health care approach.
* Nurse‐led clinics:
o Collect health history, plan, coordinate and document patient care, evaluate outcomes per PHC‐endorsed policies.
o Coordinate patient care and communicate progress to treating GPs, escalating adverse findings per PHC policies.
o Undertake clinic administration activities to facilitate nurse‐led clinics.
* Nursing leadership:
o Attend internal and external meetings where nursing representation is required.
o Delegate operational tasks (checklists, cleaning, GP rooms, etc.) to ensure efficient nursing service at the centre.
o Oversee equipment functionality, maintenance/repair, and calibration.
o Develop nursing rosters and fill vacant nursing shifts.
* Clinical administration:
o Ensure clinical areas are adequately stocked and replenish stock before depletion.
o Clean and sterilise instruments and replace those no longer in working order.
o Conduct routine activities as defined in the Treatment Room and Clinical Care Coordination checklists.
o Maintain tidy, clean and presentable clinical areas.
* Workplace Health & Safety:
o Act as a role model for WHS in practice.
o Implement WHS policies and procedures into day‐to‐day operations.
o Ensure all personnel on site, visitors and contractors adhere to WHS policies.
o Conduct workplace inspections and audits.
Minimum Requirements
Qualifications
* Current AHPRA registration as a Registered Nurse (RN).
* Continuously update personal and professional skills and knowledge to meet CPD requirements and maintain AHPRA registration.
Experience
* At least 2 years' experience providing direct clinical nursing care.
* Experience in a primary care setting and understanding of chronic disease management, prevention principles and health promotion – desirable.
* Management experience desirable.
