Based at the Warracknabeal campus, conveniently located halfway between Melbourne and Adelaide, this part-time Intake Clinician position offers the opportunity to support community wellbeing in a supportive and progressive environment.

As the Intake Clinician, you will work closely with the Intake Team to support consumers accessing a wide range of community health services. This includes initiating intake processes, creating episodes of care, scheduling appointments, and maintaining accurate client data and documentation.
About you
- Relevant qualifications as a Nurse, Allied Health Assistant, or Allied Health Professional
- Experience in person-directed care within a community setting
- Robust interpersonal and communication skills, both written and verbal
- Excellent organisation and time management skills
- A team-oriented approach with the ability to collaborate across disciplines
- Proficiency in digital systems and general computer skills
- Commitment to ongoing professional development
Advantageous/Highly Desirable
- Knowledge of Primary Care and Chronic Disease Management principles
- Familiarity with Uniti (client management system)
- Understanding of accreditation programs and related legislation
- Current Police Check, Working with Children Check and NDIS Worker Screening Check, or willingness to obtain
- Valid driver’s licence
- Immunisation history
