About the role
The Registered Nurse (RN) reports to the CEO and is responsible for the direct provision of high-quality clinical care to clients focusing on a ‘person centred’ approach to wellness. The RN will maintain collaborative relationships with clients, their families and advocates and all other internal stakeholders.
Responsibilities
* Client Care and Clinical Leadership
* Medication Management
* Wound Care
* Conduct home assessments and create individual care plans
* Deliver high quality, person centred clinical care in participants homes
* Build trusted relationships with participants, families and health teams
* Lead quality improvement initiatives and ensure compliance to strengthen aged care accreditation standards
* Guide and mentor non clinical staff
Essential Skill & Experience
* AHPRA registered clinician (Nurse, Social Worker, Occupational Therapist) with minimum 3 years clinical experience
* Experience working in a community setting as a Registered Nurse. Previous experience in care planning.
* Good Knowledge of Support at Home rules and legislation
* A sound understanding of person-centred care in an aged care setting.
* Demonstrated experience and knowledge in supporting older people and carers living with dementia.
* Effective communication skills and a proven ability to engage and influence all stakeholders, including staff, clients, and families.
* Excellent written and verbal communication skills.
* Ability to forge effective working relationships, as part of a multidisciplinary team.
* Demonstrated experience in maintaining systems and processes to meet the various legislative, compliance and reporting obligations as they relate to home care using IT systems
* Strong planning and organising skills with the ability to prioritise tasks to meet deadlines.
* Understanding of WHS practices and guidelines.
* Drivers license

* Qualifications and Registration
* Current AHPRA Registration: Registered Nurse (Division 1) with no restrictions.
* Clinical Competence: Demonstrated experience in clinical assessment, wound care management, and medication administration in accordance with legislative requirements.
2. Clinical Assessment, Care Planning, and Coordination
* Person-Centred Care: Proven ability to co-design, implement, and monitor individualised care plans that promote client autonomy, dignity of risk, and goal achievement.
* Aged Care Experience: Experience in managing client care within the aged care sector, including knowledge of the Support at Home Program and My Aged Care Provider Portal.
* Case Management: Experience in coordinating services, sourcing assistive technology/home modifications (AT-HM Scheme), and managing client budgets.
* Risk Management: Ability to identify, assess, and manage clinical and environmental risks to safety and well-being.
3. Clinical Leadership and Reporting
* Clinical Governance: Experience in identifying deterioration in client condition, following care pathways, and escalating to appropriate health professionals.
* Infection Control: Knowledge of infection prevention and control practices, including antimicrobial stewardship.
* Documentation: Experience in maintaining accurate, timely, and confidential client records in electronic systems.
4. Staff Supervision and Team Leadership
* Mentorship and Training: Experience in providing guidance, support, and training to staff and volunteers.
* Performance Management: Ability to participate in recruitment, conduct performance reviews, and verify timesheets.
* Team Culture: Ability to foster a collaborative, inclusive, and responsive team environment.
5. Communication and Interpersonal Skills
* Stakeholder Engagement: Ability to liaise effectively with clients, representatives, external health professionals, and the CEO.
* Advocacy: Experience in providing advocacy to clients to facilitate goal achievement.
* Cultural Competence: Ability to adapt communication to respect diverse cultural backgrounds and preferences.
6. Quality, Safety, and Compliance
* Regulatory Knowledge: Understanding of the Aged Care Quality Standards and relevant legislation.
* Continuous Improvement: Experience in audits, incident investigation, and implementing quality improvement initiatives.
7. Regional Context
* Innovative Approach: Ability to find creative, evidence-based solutions to meet the needs of clients living in a regional area.
