About The Role
We are seeking a dedicated Social Worker to empower older adults to live independently and safely at home. The successful candidate will work closely with the TACP team, Allied Health Team Leader, and TACP Manager. This is a new position with scope for hybrid models of care delivery.
The Aged Care Division provides clinical and non-clinical services across the Local Health District to support older people in their families, promoting independence and wellness. The role involves working within a team committed to advocacy, restorative care, and reablement, amidst a dynamic aged care environment influenced by health reforms.
