About the Role FCD Health is seeking a dedicated and compassionate Practice Nurse to join the Palmerston GP Super Clinic and Palmerston Medicare Urgent Care Clinic (PMUCC). As an essential member of our clinical team, the Practice Nurse supports General Practitioners in delivering high-quality, holistic, and culturally competent care across both primary health and urgent care services. This dynamic role offers an opportunity to work within a multidisciplinary team, providing patient-centred services including preventative health, chronic disease management, immunisation, and urgent care support. Key Responsibilities Clinical Practice Provide clinical nursing support to General Practitioners in delivering comprehensive patient care. Conduct wound care management, ECGs, spirometry, and assist in minor surgical procedures. Administer immunisations, including COVID-19 and routine vaccinations, following current protocols. Perform patient triage and support urgent care assessments and interventions. Coordinate health assessments, chronic disease management, and preventative care planning. Follow up on test results, manage recalls and reminders in line with best practice standards. Maintain accurate and timely electronic medical records using clinical software. Deliver culturally safe care, respecting patients’ values, beliefs, and backgrounds. Communicate effectively with patients and the healthcare team to ensure high-quality care. Contribute to clinical audits, infection control procedures, and quality improvement initiatives. Urgent Care Responsibilities Provide timely triage and initial assessment of patients presenting with urgent, non-life-threatening conditions. Support coordination and communication with external services as required for escalated care. Ensure appropriate documentation, infection control practices, and follow-up for urgent care presentations. Selection Criteria Essential: Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered or Endorsed Enrolled Nurse. Proven clinical experience in a general practice or acute care setting. Competency in immunisation delivery (Vaccination Certificate required). Current CPR and First Aid Certification. Current Working with Children Clearance (Ochre Card). Valid National Police Clearance. Demonstrated ability to work collaboratively within a multidisciplinary team. High-level interpersonal and communication skills with a commitment to culturally safe practice, particularly in supporting Aboriginal and Torres Strait Islander communities. Proficient in electronic health record systems and digital technologies. Desirable: Experience in chronic disease management and health promotion. Familiarity with Medicare item numbers and general practice billing procedures. Previous experience in a fast-paced or urgent care environment.
