Care Facilitator Role Overview

We are seeking a skilled and compassionate Care Facilitator to support older people in living well in their own homes.

The successful candidate will provide personalized care planning and goal-setting focused on well-living approaches, building strong relationships with clients and their families, and working collaboratively to set achievable goals.

* Visit clients in the community who require assistance at home
* Develop meaningful connections with family members and caregivers, acknowledging their needs and contributions
* Work closely with clients to establish individualized goals that are realistic and attainable


Essential Requirements

* A bachelor's degree or diploma in Community Services (Service Coordination), Case Management, or equivalent knowledge and experience
* A valid driver's license
* An intermediate level of computer proficiency
