About the Role
Shape a new chronic kidney disease care model that supports patients across their renal journey and builds consistent, high quality practice statewide. Deliver CKD education and treatment option support Guide patients through renal pathways using shared data Build and embed a CKD care model with clear outcomes Partner with renal teams to improve access and continuity
About You
You're a registered nurse with renal expertise who enjoys leading care, education and service improvement for people living with kidney disease.
Registered or eligible RN with NMBA practising certificate Experience in nephrology care and patient education Leadership skills that support teams and lead change Ability to improve care models with a person centred focus Strong knowledge of quality, safety and health legislation
