We’re looking for an experienced Care Manager for a 6-month fixed-term contract to lead and support a dedicated clinical team, ensuring the delivery of high-quality, person-centred care in a welcoming and professional environment.

Key Responsibilities
Oversee the daily clinical operations of the care home
Provide leadership, coaching, and support to the clinical team
Manage performance, training, and staff development
Ensure compliance with AN-ACC assessments and aged care standards
Conduct audits and review clinical incidents and feedback
Lead person-centred care planning and family case conferencing
Manage medication administration processes and clinical documentation
Liaise with allied health, GPs, and external care providers
About You
Current AHPRA registration as a Registered Nurse (essential)
Post-graduate qualifications in aged care (highly regarded)
Proven leadership experience in aged care or acute settings (min. 30-bed facility)
Strong clinical assessment, care planning, and documentation skills
Exceptional communication and stakeholder management abilities
Knowledge of the Aged Care Quality Standards and resident rights
