We’re looking for an experienced Care Manager for a 6-month fixed-term contract to lead and support a dedicated clinical team, ensuring the delivery of high-quality, person-centred care in a welcoming and professional environment.

Key Responsibilities
* Oversee the daily clinical operations of the care home
* Provide leadership, coaching, and support to the clinical team
* Manage performance, training, and staff development
* Ensure compliance with AN-ACC assessments and aged care standards
* Conduct audits and review clinical incidents and feedback
* Lead person-centred care planning and family case conferencing
* Manage medication administration processes and clinical documentation
* Liaise with allied health, GPs, and external care providers
About You
* Current AHPRA registration as a Registered Nurse (essential)
* Post-graduate qualifications in aged care (highly regarded)
* Proven leadership experience in aged care or acute settings (min. 30-bed facility)
* Strong clinical assessment, care planning, and documentation skills
* Exceptional communication and stakeholder management abilities
* Knowledge of the Aged Care Quality Standards and resident rights
