What we're looking for
You're an experienced professional in aged care (perhaps a Home Care Package Coordinator, Care Partner, Enrolled Nurse, or CHSP Coordinator) ready to take the next step in your career. You bring:
* Certificate IV in Community Services (or equivalent) and a current First Aid
* Proven experience in case management, service coordination, and care planning
* Strong understanding of SAH budgets and community care compliance
* Excellent communication, decision-making, and time management skills
* Confidence using digital tools (Word, Excel, calendars)
The role
* Conduct strengths-based assessments and create personalised care plans aligned with client needs and contractual obligations
* Provide person-centred case management and coordinate services for clients and carers with moderate to complex needs
* Manage budgets and ensure effective resource use
* Collaborate with health professionals and community organisations
* Maintain accurate records and meet reporting requirements
* Adapt to challenges with resilience and a solutions-focused mindset
