Role Overview
The Referral Pathway Coordinator is responsible for promoting and managing the referral pathway to ensure equitable access to the HASI program.
This role involves triaging referrals, conducting assessments, providing support services, screening referrals, making first contact calls, managing a small caseload, reviewing functions with the Service Manager, and monitoring referral pathway KPIs and data collection.
Key Responsibilities:
* Triaging referrals and ensuring timely interventions
* Conducting thorough assessments and developing targeted plans
* Providing high-quality support services and maintaining confidentiality
* Screening referrals and identifying potential risks or challenges
* Making initial contact calls and establishing relationships with clients
* Managing a small caseload and prioritizing tasks effectively
* Reviewing functions with the Service Manager and implementing improvements
* Monitoring referral pathway KPIs and contributing to data-driven decision-making
Requirements
* Current AHPRA registration as a psychologist or provisional psychologist, mental health nurse, occupational therapist (mental health), or accreditation with AASW as a mental health social worker
* Proven experience in direct service provision within psychosocial support programs or mental health services
* Strong understanding of public mental health services and relevant legislative frameworks
* Excellent analytical and problem-solving skills with attention to detail
