As a Home Care Package Coordinator, you will be an integral part of a multi-disciplinary team, dedicated to developing, implementing, coordinating, and evaluating community-based programs. Your role will support individuals who wish to continue living comfortably in their own homes with the right assistance, making a meaningful impact in their lives.

This is a part time vacancy, working 60 hours per fortnight and salary will be adjusted according to actual hours worked.
