Geriatric Care Coordinator Role
The Geriatric Care Coordinator will oversee the coordination and delivery of high-quality care services to residents in an aged care facility.
Key Responsibilities:
* Coordinate day-to-day clinical activities of care staff to ensure timely and effective delivery of care services
* Undertake direct care of residents, assessing their needs and developing individualized care plans
* Act as a mentor and educator for enrolled nurses in the areas of wound care and infection control
* Oversee the administration of medications and ensure compliance with resident care plans
* Ensure accurate documentation and maintenance of up-to-date records
The Geriatric Care Coordinator will work closely with other healthcare professionals to ensure seamless coordination of care services.
About You
* Demonstrated experience in coordinating and evaluating the quality of care services for aged care residents
* Working knowledge of accreditation standards and aged care funding tools
* Ability to lead and supervise a multi-disciplinary team to achieve excellence in delivery and management of care services
* Excellent communication and interpersonal skills, with ability to build strong relationships with staff and residents
* Ability to analyze data and contribute to continuous quality improvement initiatives
