We are seeking a Social Worker who is passionate about holistic, client-centred care, demonstrates excellent interpersonal skills, and brings enthusiasm and initiative to their role.
This is an exciting opportunity for an experienced or graduate Social Worker to join our team and make a meaningful impact on the lifelong wellbeing of individuals within our community. Our practice has developed strong referral networks and trusted relationships with GPs, community groups, specialists, and local sporting clubs, resulting in a diverse and rewarding caseload.
The Social Worker role will operate similarly to a Health Coordinator position. You will play a key role in supporting clients through care navigation, advocacy, and psychosocial support, while also assisting in the facilitation and coordination of community events, group programs, and wellbeing initiatives. You will work with clients across a range of presentations and funding systems, helping them access appropriate services and achieve their personal health and life goals. The Social Worker will be responsible for actively building and maintaining strong referral pathways within the community. This will include developing and nurturing professional relationships with GPs, community groups, specialists, local sporting clubs, and other key stakeholders to generate referrals and support a diverse and rewarding caseload.

We are looking for the right Social Worker to step into an established caseload for an immediate start (following online and onsite training). The role will involve supporting clients within our private clinic, in-home settings, and/or Medical Centres, across various funding streams including (but not limited to) Medicare, Private, NDIS, DVA, WorkCover, Insurance, and community-based populations.

The successful applicant will require:
* Experience as a private practice Social Worker is desirable, not essential
* Experience working with Defense veterans desirable
* Current AASW Accreditation
* Ability to build and maintain rapport with patients and other health professionals
* Excellent verbal and written communication skills
* Ability to work both autonomously and collaboratively within a team environment
* Competent in computer skills and ability to learn scheduling and software programs.
* Passion for ongoing learning and progression
* Driver's license
