Clinical Care Manager - Level 6
Deliver high-quality person-centered care in the community setting, aligned with Aged Care Quality Standards.
* Lead a team of professionals in managing services
* Review client intake and ensure adequate care planning, risk assessments, and initial assessments are completed at a high level
* Prioritize services for high-care clients between clinical, in-home, and contracted services
This role requires advanced clinical expertise in chronic disease management and documentation, as well as strong clinical skills and knowledge of contemporary aged care practices. The ideal candidate will have excellent communication and interpersonal skills, with the ability to build and maintain effective working relationships.
Key Responsibilities include Operations, Clinical Governance, Advocacy, Client Monitoring, Mentorship, Financial Management, Continuous Improvement, and Service Delivery from our Australind Office with regular site visits.
