Job Title: Disability Support Worker
We are seeking a compassionate and skilled Disability Support Worker to join our team.
The successful candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with customers, families and caregivers.
* Key Responsibilities:
* Provide person-centred care to help customers achieve their goals and aspirations
* Contribute information for the development of customer support plans
* Support customers with personal care, living skills, transport, social activities, medication and other needs as required
* Keep accurate and up-to-date records of customer interactions and support plans
* Develop and maintain professional relationships with customers, families, caregivers and key stakeholders
* Identify opportunities to enhance the customer experience through innovative solutions and collaboration with the team
Requirements:

* A high level of empathy and understanding of disability issues
* Ability to work independently and as part of a team
* Basic computer skills and knowledge of software applications relevant to the role
