**The Role**
The Support Coordination team is growing, and we are looking to fill Specialist Housing Support Coordinator in Geelong and surrounds.
You will be coordinating and providing support to participants to source appropriate housing or supported accommodation. You will be assisting individuals that have complex needs and working in collaboration with a multidisciplinary care team to ensure housing supports are identified.
If you are a skilled Support Coordinator that has had relevant experience in housing, Specialist Disability Housing, Independent Living Options or Flexible Living Options or have a qualification in disability studies or social work - we would like to speak to you.

**About you?**
- You will have comprehensive knowledge of disability, community service systems as well as NDIS, legislation, policy and service standards.
- Able to follow up, prioritize and communicate effectively.
- Have a warm and friendly personality and provide outstanding service to participants, as well as internal/ external stakeholders.
- Be able to engage with co-workers with a positive and can-do attitude.
- Exceptional listening skills be an effective and proactive communicator.
- Proficient literacy in Work, Excel and client record management system
**Requirements**:
- Have a current driver's license.
- WWCC or willing to obtain.
- NDIS screening clearance
- Evidence of Covid Vaccination (including booster)
- Will have a qualification in social work or Disability or experience in support coordination or relevant housing experience is highly desirable.
