North Thames Genomics Laboratory Hub
Pharmacist
Posted: 5 March
Offer description

Laurel Bank Surgery is searching for a second ARRS Clinical Pharmacist to join our team at an exciting time for the practice, as we go into a merger with the Village Surgeries Group on 1st April 2026. Laurel Bank Surgery will operate 3 rural sites across southwest Cheshire: Malpas, Tattenhall and Farndon.

As an integral part of the team, you will use and share your pharmacy expertise to support the practice in effective medicines management and optimal patient care. In particular, we seek a pharmacist who is keen to develop their in-person consultation skills, clinical assessment and management of long term conditions.

Working within a multi-disciplinary team you will work alongside the 14 doctors, 6 nurses and 4 medicines management staff in the practice, including another experienced, prescribing pharmacist.

You will be mentored by the GPs to facilitate your learning and development. You will be supported by a network of Pharmacists across the rural alliance PCN.

We are keen to hear from pharmacists of all backgrounds and levels of experience and although an Independent Prescribing qualification is desirable it is not essential.

A enhanced DBS Check is essential for the job.

Main duties of the job

As Clinical Pharmacist you will work as part of the practice multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues and improve clinical outcomes.

We will encourage you to take responsibility for areas of chronic disease management and you will undertake structured medication reviews to proactively manage patients with complex polypharmacy. You will undertake reviews of patients with specific long-term conditions that fall within your competency.

You will confidently work alongside and support a team of PCN pharmacists and technicians across the PCN practices, acting within professional boundaries. You will deliver medicines optimisation initiatives to ensure safe and cost-effective use of medicines, including work to deliver key outcomes against an ICB prescribing incentive scheme. You will contribute to quality improvement and clinical audit and support aspects of the QOF, medicines safety and antibiotic stewardship.

The role is pivotal to improving quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. The role holder will be required to complete the CPPE development pathway and complete their Independent Prescriber qualification if this is not already held.

About us
