Support Worker
Job Overview:
We are seeking a Support Worker to provide 2:1 support to consumers in their own home and community setting. The role involves developing professional relationships with consumers, their families, stakeholders and others in the community.
Key Responsibilities:
* Provide 2:1 support to consumers in their own home and community setting
* Meet the individual needs according to an established behavioural support plan
* Developing professional relationships with consumers, their families, stakeholders and others in the community
* Providing accurate and timely case notes so that others can provide a consistent experience for the consumer
* Provide support to assist consumers to work towards their individual and NDIS goals
Requirements:
* Proven experience supporting individuals who display extremely challenging behaviours as a result of their mental ill-health and/or disability
* Experience working with Aboriginal people and communities
* Prior knowledge and experience supporting people with complex mental health where challenging behaviours are common
* Patience and emotional resilience with effective listening skills & an adaptable communication style
* Capability to advocate for others and build sustainable support networks
* Confident ability to use IT systems across computer and mobile devices
