We're expanding and are looking for a dedicated Nurse to join our Access Aged Care team in the Ingham Shire.

What You'll Do
* Provide clinical management and coordination across local aged care facilities.
* Conduct regular resident reviews and support person-centred care plans.
* Work closely with a multi-disciplinary team, supported by a Nursing Regional Manager and admin staff.
* Build strong relationships with residents, families and facility partners.
* Use your clinical knowledge and IT skills to ensure quality outcomes.

What You'll Bring
* Diploma of Nursing and AHPRA registration.
* Aged Care experience (leadership experience highly regarded).
* Strong organisational, problem-solving and communication skills.
* Confidence with IT and documenting resident needs.
* A driver's licence and reliable vehicle.
