This role is part of a dynamic home care team, offering variety across community clients, collaboration with a multidisciplinary team, and the chance to create meaningful interventions for people living with dementia, chronic conditions, or age-related challenges.
Key Responsibilities
* Deliver individualised occupational therapy services to home care clients.
* Conduct assessments, respond to referrals, and develop tailored therapy programs.
* Work closely with Therapy Assistants and the broader allied health team.
* Collaborate with families, carers, and support networks to promote independence and wellbeing.
* Maintain accurate clinical documentation and ensure compliance with WA aged care standards.
* Contribute to continuous improvement and implement evidence-based, person-centred therapy.
About You
* Full, unrestricted AHPRA registration as an Occupational Therapist.
* Experience in home care, community therapy, or aged care highly regarded.
* Strong communication, relationship-building, and problem-solving skills.
* Ability to manage a varied caseload and adapt to client needs.
* Customer-focused, with the ability to work effectively across a broad range of stakeholders.
* Current NDIS Worker Screening Check or willingness to obtain.
* Commitment to vaccinations and upholding organisational values.
