Job Title: Social Worker
The Changing Futures Team is seeking a Social Worker to join their team in Brighton (UK). This is a unique opportunity to make a real difference in the lives of individuals with multiple complex needs.
Key Responsibilities:
* To employ a trauma-informed, person-centred approach to support those experiencing multiple complex needs.
* To cooperate with a multi-disciplinary team to provide efficient and effective service provision, fulfilling statutory duties under the Care Act and Mental Capacity Act to ensure exceptional care for all clients.
* To contribute to identifying blocks and barriers within the system, using innovative thinking to enhance service delivery, and engage in direct client work, conducting thorough assessments and delivering tailored interventions to support positive outcomes.
