About the Role
We are seeking an enthusiastic and motivated Client Coordinator to join our team, overseeing and working at our Respite Cottage.
In this role, you will be responsible for assessment and coordination of responsive and flexible quality care that promotes choice and person-centered approach.
* Deliver high-quality support and services to participants, ensuring their needs are met in a professional and caring manner.
* Monitor and increase service delivery hours to meet Key Performance Indicator (KPI) expectations.
* Work under Aged Care standards, Department regulations, policies and procedures.
* Ensure participant reviews and care plans are up-to-date and completed within KPI timeframes.
* Evaluate incidents, complaints, feedback, and escalate directly to relevant stakeholders.
* Complete service agreements and budget tools with participants.
* Identify deterioration and liaise with health professionals and other agencies.
* Organize excursions/outings in consultation with clients' choices and interests.
* Plan and evaluate day programs to meet individual client care needs.
* Oversee meal planning to ensure dietary requirements are met.
