Key Responsibilities
* Coordinate and monitor delivery of services under Home Care Packages (HCP)/Support at Home
* Support intake, onboarding for new clients
* Manage and support a team of community care workers
* Liaise with families, GPs, allied health professionals
* Ensure compliance and quality as per standards
* Optimise budgets and resources
Qualifications
* Experience working in Home/Community Care as a Case Coordinator or Home Care Manager
* Experience in managing and coordinating Home Care Packages HCP/CHSP or NDIS
* Excellent engagement and communication skills
* Amazing customer service, friendly approach, compassion and values focused
* Current driver's licence
* National Police Clearance
