About the Role
Working within our Mobile Intensive Rehabilitation Team, you will be responsible for providing leadership in the development and implementation of the Queensland Care Coordination model to support people with severe mental illness and complex care needs. You will be focused on improving the coordination of services between government, non-government and private providers. To succeed, you will have an in-depth knowledge of current practices, philosophies and trends in the delivery of mental health services. You will have the confidence to successfully negotiate with a broad range of stakeholders to ensure that outcomes are guided by our vision, Caring for our Communities: Healthier Together.
