We are seeking a dedicated social worker to empower older adults to live independently and safely at home.
This is an exciting opportunity to work with our multidisciplinary team, delivering clinical and non-clinical services that promote independence and wellness for older people in their families.
The successful candidate will work closely with the TACP team, Allied Health Team Leader, and Manager, providing comprehensive care and support to our clients.
Key responsibilities include:
* Empowering older adults to maintain their autonomy and dignity
* Collaborating with healthcare professionals to deliver patient-centered care
* Providing education and support to families and carers
Requirements:
* Degree in social work or related field
* Experience working with older adults and their families
* Ability to work collaboratively as part of a multidisciplinary team
