Job Description
The Practice Nurse will play a pivotal role in delivering high-quality patient care to our homeless and vulnerably housed population in Brighton& Hove. As a skilled nursing professional, you will be responsible for providing compassionate and empathetic care to patients with complex needs.
Key Responsibilities
* Provide nursing care to patients as needed, including treatment, screening, and preventative care.
* Work collaboratively with the wider team of doctors and nurses to support and care for patients through assessment, diagnosis, and management of their health needs.
* Manage chronic diseases such as diabetes, COPD, asthma, and hypertension, including maintaining registers and constructing care pathways as appropriate.
Required Skills and Qualifications
* Effective communication and interpersonal skills.
* Knowledge and understanding of current national developments and trends in healthcare and practice development.
* Experience of Microsoft Office software.
* Understanding of clinical coding in general and Read coding in particular.
