* Pay details-Salary packaging: $ $ % superannuation and salary packaging up to $15,900 per year

About the Role:

The role is responsible for delivering client focused, outcome driven case management to young people, and families within the Permanency Support Program (PSP). You will work collaboratively with families and key stakeholders to support safety, stability, and long-term permanency outcomes.

Key Responsibilities:

As the Case Manager you will:

* Prepare high quality reports and case notes that are accurate, timely, objective, and aligned with organisational and legislative requirements.
* Engage effectively with stakeholders including families, significant others, service providers, and community partners to support positive outcomes for young people.
* Build and maintain strong relationships with families to enhance collaboration, transparency, and shared decision making.
* Coordinate and support the wellbeing of young people with complex needs through structured planning, daily organisation, and proactive intervention.
* Support the team in advocacy by liaising with, external agencies, and internal stakeholders to ensure young people receive appropriate resources and services.
* Work confidently with foster carers to support young people with varied needs while maintaining professional and emotional regulation.
* Collaborate with the care team to deliver consistent, trauma informed, and person centred care.
* Contribute to risk management and safety planning by identifying risks, participating in safety planning, and implementing strategies that promote stability and wellbeing.
* Bring operational reliability, including active participation in the on‑call roster to support critical services.

What you will bring:

* Relevant qualifications in the Human Services field are required. Minimum Diploma of Community Services or equivalent; degree qualifications preferred.
* Two years' experience working as a Case Manager, within Community Services field.
* Mandatory compliance requirements including a valid NSW Working With Children Check, current Driver's Licence, current First Aid Certificate, and registration under the NSW Residential Care Worker Register.
* Strong report writing and case noting capability with the ability to document clearly, objectively, and in line with best practice.
* Excellent communication and engagement skills that support meaningful relationships with families, young people, and the care teams.
* Experience supporting young people with complex needs and the resilience to work effectively in high pressure or rapidly changing environments.
* A collaborative mindset with the ability to work cohesively within a care team and contribute to shared goals.
* Confidence in risk assessment and safety planning, including the ability to recognise early warning signs and respond appropriately.
* Strong organisational skills with the ability to prioritise competing demands while maintaining a focus on wellbeing and therapeutic outcomes.
* Advocacy skills and experience liaising with funding bodies to ensure young people receive the supports they require.
* Commitment to trauma informed, youth centred practice and a genuine passion for improving outcomes for vulnerable young people.
