About the Role
We are seeking a passionate and skilled Care Coordinator to join our team in Wagga Wagga, NSW (Wiradjuri Country). As a Care Coordinator, you will work closely with clients, GPs, community nurses, specialists, medical staff, and Aboriginal Medical organisations to provide high-quality care for individuals managing chronic diseases and complex health challenges.
Your Key Responsibilities
* Coordinating comprehensive care plans for individuals with chronic conditions and complex health needs.
* Collaborating with patients, families, carers, and healthcare professionals to ensure seamless care coordination and enhance patient outcomes.
* Working closely with local health teams to streamline services and improve access to vital resources.
* Communicating effectively with patients, carers, and the community to ensure understanding and active feedback.
About You
To thrive in this role, you will need:
* Registration as an Enrolled or Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) and/or as a qualified Aboriginal Health Worker.
* Proven experience in conducting comprehensive assessments, developing care coordination plans, delivering safe and responsive care, and evaluating outcomes.
* Strong understanding of chronic condition management and ability to navigate complexities effectively.
* Excellent communication and interpersonal skills, with a commitment to cultural safety and engagement with diverse communities.
* A person-centred approach, working collaboratively with individuals and families to ensure their voice is heard in care decision-making.
