We are seeking a dedicated and compassionate individual to fill the role of Community Support Partner.
This position involves providing support and working in partnership with individuals living with disability to enable them to live within their community in the way they desire.
* The successful candidate will collaborate with customers to develop personalized support plans that incorporate their identified wellbeing needs, strengths, and goals.
* Support will be provided to customers to enhance their independence by assisting with skill development, transportation, social activities, financial management, appointment attendance, cooking, medication, and personal care, as well as other necessary support.
* The Community Support Partner will encourage and support customers to participate in activities of their choice, such as social, leisure, sporting, educational, or employment activities.

Requirements
* A minimum Cert III in Disability, mental health, Peer Work, or equivalent qualification.
* A valid NSW Drivers License (Class C), with the ability to drive large vehicles and/or vans.
