**Job description**:
**DFP Recruitment is seeking experienced Senior Complex Case Managers (EL1 level) and Complex Case Managers (APS6 level) to join our client who is a Federal Government department.**
**The team**
This team is the gateway to all supports and provides a single access point for advice, assessment and responses to urgent, high risk or escalated client matters identified by any internal or external stakeholders.
**The role**
As a Complex Case Manager, you will be responsible for supporting the provision of high quality, client-centric, trauma informed case management practice. You willl manage a case load of complex clients and provide support and advise as required to other Complex Case managers. Uou will utilise your experience gained in case management/clinical settings, working with clients with mental health conditions, vulnerabilities and/or complex co-morbidities. You will be experienced in considering the social determinants of health and wellbeing and using the domains of wellbeing.
**Some responsibilities may include but not limited to**:
- Managing clients who are presenting with more challenging behaviours.
- Conducting assessment of client referrals made where mental health issues are evident.
- Recording appropriate case notes and other records in a case management system.
- Negotiating client claim outcomes with claims management areas of DVA.
- Conducting urgent risk assessments with clients where necessary.
- Providing advice and support to non-clinical staff in the team undertaking assessment of client referrals.
- Undertaking calls to clients where required to offer assistance or obtain further information, where this contact is best managed by a mental health professional.
- Linking clients to appropriate mental health services, such as Open Arms.
- Occasionally providing short term case management intervention to link clients with necessary services for ongoing support.
**To be successful you must have the following**:
- Tertiary qualification in a relevant health field including Nursing, Psychologist, Occupational Therapy or Social Work.
- Registration with AHPRA or AASW is highly desirable
- 2+years' experience working as a case manager, or equivalent clinical role
- Extensive experience working within a mental health or community case management setting, including working with people who present with complex mental health conditions.
- Experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients in community mental health teams.
- Excellent written and oral communication skills, including capacity to liaise with and develop productive relationships with other organisations
- Experience and interest in triage, assessment, and case management via telephone.
- Must be an Australian Citizen.
**Desirable expertise**:
- Experience in working with military or veteran mental health services.
- Expertise in supporting clients experiencing the impacts of substance abuse, mood, and anxiety disorders (including PTSD).
- Experience with and understanding of the principles of Trauma Informed Care.
- Experience working with clients with challenging behavioural presentations.
