* This role requires a Working with Children Check.
* Are you able to obtain and maintain that clearance?
* As part of the Working with Children Check, you will be required to provide the authorities with proof of your identity.
* That will include identity documents such as a full birth certificate, current Australian passport, valid Australian visa, Australian citizenship certificate or official immigration record.
* You will also need to provide a primary community document such as a driver's licence or proof of ID or other similar document.
* You will also need to provide 2 supplementary documents such as a Medicare document or evidence of enrolment to vote or the like.
* Are you able to obtain or provide those documents or similar ones required to the authorities?
* If you have changed your name, you will need to provide documents to the authorities about that.
* You will need to provide linkage documents such as a marriage certificate or a revised and reissued Australian birth certificate.
* Are you able to provide or obtain all relevant documents or similar ones to the authorities?
* The law requires us to check a person's prohibition status before we employ them.
* If you are offered a role, you will need to tell us if you are prohibited from providing education and care or aware of any enforcement action that the regulatory authorities may have against you.
* We may also ask for a signed declaration confirming your prohibition status.
* Are you willing to provide that information and sign the declaration?
