About The Role
The Community Health Enrolled Nurse is an integral part of the multidisciplinary healthcare team providing direct nursing care to patients, both in the community setting and Ambulatory Care setting.
The role works closely with the Chronic Care, Wound & Stoma Care, Palliative Care and community based services to support holistic and patient centred care that assists people to live well at home, avoid preventable hospital admissions, and supports earlier safe discharges.
