ISCP Care Coordinator - ATSI Identified PositionIWC is an Aboriginal community-controlled and multi-accredited health and wellbeing organisation that delivers services without discrimination to Indigenous and non-Indigenous peoples in Bundaberg and the Wide Bay Burnett.ABOUT THE POSITION:The Indigenous Stepped Care Program (ISCP) Care Coordinator supports the Stepped Care Mental Health program within IWC.
The ISCP will provide Aboriginal and Torres Strait Islanders with or at risk of mental health illness tailored supports.DUTIES:Arrange services for clients according to care plans in consultation with health professionals.Prepare brokerage request forms and correspondence to stakeholders, including clients and care team members.Liaise with external stakeholders to promote referral pathways and enhance services.Coordinate activities to meet operational requirements.SKILLS & EXPERIENCE:Attention to detail with high accuracy.Professional phone manner.Maintain confidentiality.Record keeping and adaptability to new systems.KEY REQUIREMENTS:Certificate III/IV in Primary Health Care or willingness to obtain.Current driver's license or ability to obtain.First Aid Certificate or willingness to obtain.Blue Card (Working with Children Card QLD) or willingness to obtain.Federal Police Check or willingness to obtain.
*No Card, No Start legislation requires the applicant to hold a valid Blue Card before starting.
IWC covers the application cost.SALARY PACKAGE:Salary Sacrifice Packaging up to $15,990 per annum, increasing take-home pay by approx.
$2,000 after probation.Meals and Entertainment Package up to $2,600 per annum (post-probation).9-Day Fortnight (full-time staff).
Superannuation at 12%.
Annual Leave Loading of 17.5%.
