Care Coordinator Clinician
FocusOne Health seeks a skilled care coordinator clinician to deliver therapeutic mental health services and coordinate integrated care. Key responsibilities include delivering high-quality mental health services, working closely with healthcare providers, and contributing to the delivery of excellent patient outcomes.
The successful candidate will have relevant qualifications in Social Work, Occupational Therapy, Mental Health Nursing, or Registered Psychology.
Key qualifications include expertise in mental health assessments and therapeutic services, as well as experience working with diverse populations and developing effective treatment plans.
