Our community nurses provide a range of services to Westbury, Hadspen, Longford, Perth and surrounding areas. Our Community Nursing Service works closely together to focus on delivering excellence in healthcare to achieve improved outcomes for our community members.The Role:We are seeking a Registered Nurse to strengthen health outcomes through the provision of safe quality, clinically appropriate nursing care in partnership with clients, their families and other health professionals within the defined community practice area.You will need:In collaboration with members of the healthcare team plan, implement and evaluate patient/client care.Interact effectively with patients'/clients' families and other health team members to facilitate the provision of optimum patient/client care.Active involvement in maintaining and continually improving the quality of patient/client care.
