We are expanding our team and seeking a passionate Social Worker who understands the NDIS landscape and is dedicated to advocating for participant choice and control.
About the Role
As an NDIS Social Worker, you will work closely with participants, their families, and multidisciplinary teams to provide holistic support that aligns with NDIS goals. You'll play a key role in assessment, capacity building, advocacy, and linking participants with appropriate services.
Key Responsibilities
* Conduct psychosocial and functional assessments to inform NDIS planning and goal development
* Support participants to navigate the NDIS, understand their plans, and maximise their funded supports
* Provide capacity-building interventions, case management, and ongoing participant support
* Collaborate with internal and external providers to coordinate services
* Advocate for participant needs, ensuring their voice and preferences remain central
* Maintain accurate case notes and documentation in line with NDIS and organisational requirements
* Work within the NDIS Practice Standards and Quality & Safeguarding Framework
Qualifications & Requirements
* Bachelor's or Master's degree in Social Work
* Experience working within the NDIS or disability sector (highly desirable)
* Strong understanding of NDIS processes, terminology, and participant rights
* Excellent communication, problem-solving, and rapport-building skills
* Ability to work autonomously and manage a diverse caseload
* Current Working With Children Check, NDIS Worker Screening Check, and valid driver's licence

Pay: $45.00 – $50.00 per hour
