Key Responsibilities
* Deliver preventative health and chronic condition management consultations to Veteran patients
* Cover areas including joint health, diabetes education, skin checks, chronic pain management, and mental health referrals
* Participate in multidisciplinary case conferencing, collaborating with Social Workers, and Allied Health providers (Physio, Exercise Physiology, Podiatry, Dieticians etc.)
* Conduct care reviews and continuity checks to ensure comprehensive, coordinated care

About You
* Vocationally Registered GP with current AHPRA registration
* Confident in both in-person and Telehealth consultations
* Collaborative, reliable, and proactive in multidisciplinary teamwork
* Passionate about holistic, Veteran-focused care
