You will work collaboratively with a multidisciplinary team to provide evidence-based practice and develop your knowledge, skills, and experience in the field.
The successful candidate will have the ability to assess client needs, identify meaningful goals, and develop practical strategies to achieve these goals.
Key Responsibilities:
* Support clients, their families, and/or carers to identify needs through assessment and person-centred planning
* Develop, deliver, and evaluate evidence-based support plans within complex support environments
* Ensure individual plans reflect clients' goals, passions, and strengths
* Build the knowledge, capacity, and strengths of those who support our clients – family, carers, friends, etc
