Where you'll be working
You'll be an integral part of the Specialist Intellectual Disability Health Team (SIDHT) in Western Sydney Local Health District (WSLHD) - a new outpatient service designed to improve health outcomes for adults with intellectual disability and complex medical needs. The team provides holistic assessment, coordinated care planning, and specialist input to address unmet or unresolved health issues. By working closely with hospitals, GPs, and community providers, the service helps people access the care they need and ensures their health concerns are recognised, understood, and managed effectively.

Essential Criteria
* Qualification: Tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW)
* Registration in line with Award requirements: Membership of the AASW
Selection Criteria
* Relevant qualifications as a Social worker and eligible for registration with the Australian Association of Social Workers (AASW).
* Extensive post graduate clinical experience providing evidence-based assessment and treatment for consumers and their families.
* Ability to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
* Demonstrated ability to work effectively within a multidisciplinary team to accomplish service goals and improved consumer care and outcomes.
* Highly developed written and verbal communication skills and competence in the use of information technology systems.
* Ability to initiate, lead, complete and share quality improvement initiatives and services evaluation processes and lead ongoing quality improvement activities with other staff.
* Extensive contemporary experience and knowledge of social work practice, as well as demonstrated understanding of the principles of intellectual disability informed care and its applications when working with consumers and families/carers.
