About the Role

We are seeking dedicated, compassionate and person-centred clinicians to join the team as a Clinical Nurse Consultant Grade 2 within the Community Assessment Unit (CAU) at Prince of Wales Hospital.

The Clinical Nurse Consultant is a Registered Nurse responsible for providing clinical leadership and coordination within the CAU, which delivers an innovative, community-facing model of care with strong integration across community-based services and collaboration with General Practitioners and primary health care providers.

The unit is designed to reduce Emergency Department and inpatient length of stay through rapid assessment, timely diagnostics and early intervention for patients presenting with undifferentiated, complex, chronic and non-critical medical conditions.

Patients can be referred directly to the CAU from the ED and triage, outpatient clinics, consultant rooms, Nurse Practitioners and General Practitioners. The average length of stay in the CAU is approximately 48 hours.

Responsibilities

As Clinical Nurse Consultant, you will provide clinical leadership and coordination within the Community Assessment Unit. Key responsibilities include rapid assessment, timely diagnostics, early intervention, and collaboration with community-based services and primary health care providers to reduce ED and inpatient length of stay.

Qualifications & Selection Criteria

* Current Authority to Practice as a Registered Nurse with the Nursing and Midwifery Board of Australia.
* Minimum five years full-time equivalent post-qualification experience, with at least three years full-time equivalent experience in an acute/critical care specialty field and postgraduate qualifications relevant to the field of practice.
* Demonstrated advanced skills in clinical assessment, clinical management and advanced life support.
* Effective communication and interpersonal skills across a variety of mediums and an understanding of the practice development framework.
* Proven ability to implement changes and develop direction in clinical practice by utilising evidence-based research and developing clinical practice guidelines.
* Strategic and clinical service planning skills, including an understanding of organisational behaviour, change management and cultural change processes.
* Leadership, clinical teaching and competency in contemporary information systems, together with experience developing specialty education programmes.
* Clinical expertise in client-centred consultancy with the ability to work independently.
