About the Role
We are seeking a skilled Case Management Specialist to join our team. As a Case Management Specialist, you will be responsible for coordinating and attending relevant appointments, including medical, school, and family contact.
Your Key Responsibilities:
* Develop and review case plans with a strengths-based, trauma-informed approach;
* Provide therapeutic case management for children and young people in care;
* Coordinate and attend relevant appointments, including medical, school, and family contact;
* Maintain high-quality, factual documentation that meets legislative and ethical standards;
* Engage with carers and families to promote stability and connection;
* Work collaboratively within a multidisciplinary team and comply with all care standards and regulations;
* Participate in reflective supervision, peer debriefs, and organisational learning;
About You:
We are looking for someone who has a Bachelor degree in Social Work, Psychology, Teaching, Community Services, Counselling, or related areas; understands the realities of working in child and family welfare, and brings a trauma-informed, culturally sensitive, and therapeutic mindset.

Job Requirements:
We require applicants to have a minimum of 12 months of experience working as a Therapeutic Case Manager, and meet all mandatory compliance requirements, including a current Working With Children Check, National Criminal History Check, and valid Driver's Licence.
