**Role Statement**
Grampians Watch is an innovative non-admitted program under Grampians Health. CHRH provide Health Coach services under a contractual arrangement, for clients (over 18 yr.) within the CHRH catchment. The program is designed to support clients in the community via clinical assessment, health education, Allied Health referrals and continued phone support, typically 3-6 months. This differs from HARP, which uses care coordination and a multidisciplinary team approach. The purpose of the program is to assist at risk people from readmission to hospital.
The Health Coach is a Clinical Nurse who is an expert in providing health information and advice about a range of chronic and complex medical conditions to clients on the Grampians Watch program. The Health Coach helps people achieve their health and wellness goals, advocates for preventative care and offers continuous support and motivation over a period of time. The Health Coach works closely with the Grampians Watch team, the client and other healthcare providers to ensure a holistic approach to health.
**Details of Appointment**:
Part-time fixed term position working 16 Hours per fortnight until the 30 June 2026.
**Salary**:
Dependent on experience, as per award + superannuation.

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019. Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
