About Us

Nepean Blue Mountains Local Health District (NBMLHD) is an exceptional environment to advance your career while enhancing your skills and expertise. The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).

The Supportive and Palliative Care Manager, Primary Care and Community Health will lead, direct and manage the Supportive and Palliative Care stream across NBMLHD. This includes strategic and operational planning and governance requirements, to enable safe, effective and appropriate services to be delivered to people with life-limiting conditions.
The Supportive and Palliative Care Manager has responsibility for the quality of client services and risk management strategies and ensures accountability for professional practice and patient safety in the Supportive and Palliative Care stream.

What you will bring to the role

* Relevant tertiary qualifications in a related clinical discipline and/or extensive experience in applying clinical knowledge to the development of clinical models of care for people with life-limiting conditions.
* Demonstrated extensive leadership experience in a health setting.
* Highly developed interpersonal, communication and leadership skills.
* Demonstrated ability in leadership and management of multidisciplinary staff to deliver efficient and effective integrated models of care that are person-centred and consider diversity, wellness, reablement and patient flow within the whole of health and social service systems.
* Demonstrated high-level skills in policy and planning development, stakeholder engagement, consumer and carer consultation, and the ability to influence service planning and delivery across a complex health service environment.
* Demonstrated ability to manage the physical, financial and human resources of a large and complex service delivery organisation to ensure the resources available are appropriately utilised.
* Ability and experience in improving health service delivery based on change management and quality improvement processes.
* Current driver's license and willingness to use for work purpose travel.
