Job Summary
We are seeking a skilled Support Professional to provide person-centred support to residents with complex needs, including co-morbidities like psycho-social disabilities and high physical support requirements.

About the Role
The successful candidate will be responsible for providing tailored services to meet individual needs, such as personal care, domestic duties, medication administration, behaviour support, skills development training, and social support.
Maintaining up-to-date knowledge of participants' needs and care plans, including changes to their care needs, is also a key aspect of this role.

Key Responsibilities
* Provide support services that cater to individual needs, including transferring and hoisting, personal care, domestic duties, medication administration, behaviour support, skills development training, and social support.
* Maintain accurate knowledge of participants' needs and care plans, including any changes to their care needs.
* Facilitate community inclusion and participation by supporting participants to establish and maintain relationships and interests through access to community activities and services.

Requirements
* A qualification in disability, mental health, or a related field (desirable).
* At least 2 years' experience supporting people in a related sector, including those with disabilities, mental illness, community services, aged care, residential, or rehabilitation environments.
* Experience supporting participants with high needs and exceptional communication skills, including non-verbal cues interpretation.
* A clear understanding of professional boundaries and restrictive practices, with the ability to set priorities, meet deadlines, and work under pressure.
