Mental Health Nurse/Clinical Care Coordinator

Role Description
This is a contract, preferable on-site role in Toowoomba, QLD for a Mental Health Nurse/Clinical Care Coordinator. The Mental Health Nurse/Clinical Care Coordinator will be responsible for managing client appointments, coordinating care plans, short-term therapeutic interventions/assessments, and triage and connection. The role will include liaising with clients, healthcare providers, and other stakeholders to ensure smooth service delivery. Additionally, the Mental Health Nurse/Care Coordinator will be tasked with maintaining client records, supporting the clinical team, and ensuring compliance with relevant regulations and standards.
Qualifications
- Registered Nurse with experience in Mental Health
- AHPRA registered with Mental Health Credentialing or willingness to work towards Credentialing.
- Excellent organizational, time management, and administrative skills
- Strong interpersonal and communication skills
- Experience with client management and care coordination
- Familiarity with healthcare regulations and standards
- Ability to work independently and collaboratively in a team environment
- Proficiency in using office software and client management systems
- Knowledge and experience with evidence-based mental health clinical interventions
- Bachelor's degree in Healthcare Administration, Psychology, or a related field is preferred
