Care Navigator Role

* Collaborate with healthcare teams to develop personalized care plans for clients
* Provide empathetic support to clients and their families, promoting cultural safety and understanding
* Engage with the community to raise awareness about mental health services and promote healthy relationships
The ideal candidate will possess excellent communication skills and be able to build strong relationships with stakeholders. This role requires a Certificate IV in Mental Health and identification as Aboriginal or Torres Strait Islander.
