About the Role
As a Care Coordinator, you will play a vital role in planning and managing care services for clients receiving in‐home support. You'll work closely with field staff, clients, families and health professionals to deliver services that truly make a difference.
Key Responsibilities
* Conduct initial assessments and develop person‐centred care plans.
* Coordinate and schedule care services with accuracy and efficiency.
* Support carers with guidance, resources and regular check‐ins.
* Maintain compliance with care standards and documentation.
* Ensure clients feel heard, respected and supported at all times.

Qualifications
* Certificate IV or Diploma in Aged Care, Disability, Community Services or similar (preferred) with a minimum of 2 years' experience.
* Knowledge of Home Care Packages (HCP) and/or NDIS.
